When machines experience a failure our first approach is to collect the technical information and failure history. We then examine the failure evidence and contruct, where possible, the sequence of the failure events. This leads to a number of possible causes, which are examined in more detail. By means of scoping calculations and analysis we are able to eliminate many of the possible causes of failure and focus on the key areas for a more detailed micro-analysis.
Failure projects invariably benefit from ‘hands on’ investigation on site, although we are sometimes able to give advise via photographic and descriptive evidence alone.
